New Patient InfoWhat should I bring to my first appointment?

  • Documentation: The prescription from the doctor (if you have one).
  • Expedite your initial visit by downloading and filling out our Patient Registration Forms ahead of time.
  • Your health insurance card and a photo ID.
  • Any additional reports related to the condition. (Ex. MRI, X-ray, etc.)

What should I expect at my first treatment session?
First a Physical Therapist will evaluate your condition and formulate a treatment plan to meet your goals. The first session will last between 45 minutes and one hour, and following visits can range from 30 minutes to an hour in length.

What should I wear to my appointment?
Wear comfortable, loose clothing and tennis shoes. We have a changing room available for your convenience. Please be conscious of wearing loose clothing around the area specific to your injury. Example: neck or shoulder injury, wear a tank top…knee or hip injury, wear shorts, etc.

What insurances accept Back In Motion Physical Therapy?
Insurance companies such as Aetna, The Alliance, Blue Cross/Blue Shield, Cigna and others allow you to see a physical therapist without a physician’s referral. And if you have an insurer that we don’t accept, or if you don’t have insurance, we also offer reasonable cash rates. Please contact us at (608) 838-7232 regarding your specific insurance.

We have an easy-access, ground-level parking lot with handicap parking spots in front of our clinic. We are just off of US Hwy 51 with access from Dale Rd. and Hwy 51. Find us on a map

No-show / Cancellation Policy
We understand that life can throw us curve balls every now and then. If you need to cancel and/or reschedule your appointment, all we ask is that you try and call 24 hours before your appointment time to let us know. Because our time is just as valuable as yours is, we do charge a $20 fee if you are a no-show and did not call ahead.
